Player Registration – Acting – Thurston Community Players

A separate player registration form must be completed for each new and returning player who will be involved in the show as either an actor or a non-actor. Past user data was not able to be transferred during the system upgrade.

The information in this form will be used only for casting and job assignment and in cases of emergency. The only exceptions are player name, email, phone, and grade, which will be included in an online directory available only to players logged into the website.

WAIVER

GENERAL LIABILITY RELEASE This general release is intended as a waiver of liability for Thurston Players, its Board of Directors, members, administration, staff, and volunteers (hereinafter, collectively the “The Thurston Players”) for all acts or omissions arising from any and all activities or operations related to the Thurston Play. Such activities may include (but are not limited to) auditions; rehearsals; performances; preparation for auditions, rehearsals, or performances; clean-up; set-up; and all duties of parent and child volunteer positions, including but not limited to producers, directors, child supervisors, set design and implementation, writers, script committee, actors, historians, choreographers, musicians, props, sound, lighting, promotion, advertising, ticket sales, stage crew, concessions, house manager, cue runners, publicity, costume design and implementation, make-up, photography, video, and all other functions of a school play not specifically stated in this document.

Participant and parent or guardian of participant acknowledges that there are risks involved in all such activities and in signing this waiver, the parent, guardian, or adult participant acknowledges such risk and agrees to indemnify and hold The Thurston Players harmless for any injury or damage arising out of the play-related activities, unless such claim is caused solely by The Thurston Players’ gross negligence or intentional acts. The parent, guardian, or adult participant further agrees that this General Release is given in exchange and in consideration for the services provided or to be provided by The Thurston Players, including the participation in the activities described above.

CONSENT AND RELEASE FOR USE OF IMAGE, LIKENESS, AND VOICE I give permission to the Thurston Players to use, without charge and without reservation, all or a portion of my performance(s) in the play, including my image, likeness and my voice in publishing and promoting the play. I acknowledge that editorial changes may be made as deemed suitable by the Thurston Players. Whether or not changes are made, I waive any rights of action I may have and release the Thurston Players from any and all claims I may have arising from use and publication, including any rights to sue for defamation or violation of rights of privacy or rights of publicity.

MEDICAL RELEASE I understand and agree that Thurston Players, its Board of Directors, and its members and volunteers are not responsible for any injury or any illness that occurs to me or my child while practicing, rehearsing, preparing for, performing, or doing any other function related to the Thurston Play.

I agree the Thurston Play is NOT a drop-off after school activity. I am solely responsible for watching and caring for my child while he or she participates in any function related to the Thurston Play. I further understand and agree that the Thurston Players, its Board of Directors, and its members and volunteers are not responsible for watching or caring for my child. I agree to hold Thurston players, its Board of Directors, and it members and volunteers harmless and to indemnify them should anything occur to me or my child while he or she participates in or volunteers for any function related to the Thurston Play.

Waiver of Liability *

As parent(s) or guardian(s) of a play participant, or adult participant(s) and volunteers, I (we) understand and agree that execution of this document constitutes a full release and waiver of liability as stated above and authorization for my child, child under my guardianship, and/or me to participate in The Thurston Play.
